This is a planned service offered to those who would have just under gone surgery and need post-operative surgery support in their own homes. This service can also be offered as a planned service for those who already have a care package in place and have a pre-booked hospital admission. It would normally be planned ahead of your hospital admission. This ensures that your plan of care is in place from the moment you are discharged from hospital.
The services available for our Hospital to Home and Post-operative Care are as follows:
Our hospital to home service can support you with your transfer from hospital. We can arrange for a Nurse or Care Support Worker to meet you at the hospital (This could be your regular nurse or care worker if we are already providing services to you) and suitable transport can be arranged on your behalf if required.
We can liaise with the hospital Discharge team to ensure that any required equipment has been installed in your home, e.g hoisting equipment. We can prepare your home for discharge from hospital (ensuring heating, electricity and water are working, and there’s a sufficiently-stocked pantry).In addition, we will ensure that a full risk assessment is carried out in your home to make sure it is completely safe for your return.
Following your discharge from hospital, we will work closely with you to make sure our staff are able to provide care based on the hospital team’s recommendations. We will train our staff to accommodate any special requirements that you may have.
Care at short notice
At times hospital discharge can happen very quickly meaning that hospitals are not always able to give substantial notice before arranging your discharge. We are able to set up care at short notice, our service can often commence within 48 hours of you getting in touch with us, or sooner, if need be. We work closely with your G.P, hospital Doctors, Nurses, Discharge Teams and Social Workers to ensure we are speaking on the same terms and no information is lost in transit. This service is not only limited to hospital discharges but available to those needing a care package at short notice or in an emergency in their own home.
Our other services included within this package include:
- Helping to arrange regular food or meal delivery.
- Home adaptations such as grab rails, key safes, plumbing and electrics / Arranging someone to move furniture / Finding a gardener.
- Assisting with arranging and setting up telecare
- Your short-term and ongoing care needs
